The participant acknowledges/consents that his/her personal data, including but not limited to their name, gender, telephone/mobile number and email address/es, as submitted by the participant shall be collected and processed by FAVOR CHURCH through attendance sheets, feedback forms, survey forms, photos, video/audio recordings, and other data collection and monitoring & evaluation tools for documentation purposes. This is in adherence to the Philippines Data Privacy Act of 2012, and Favor Church Data Privacy Policy.
